What exactly is sent?

If you opt in: meaningful game actions, score and progression, failures and retries, pause/resume, session timing, approximate frame rate, a coarse device type, and a random ID on this device that links shared sessions. If you are signed in, a shared session may also link to your Portal player for leaderboard eligibility; eligible display names can appear on that game's leaderboard. Attaching your account name to explicit feedback is a separate opt-in. Shared records are kept indefinitely; there is no self-serve erase yet.
